Referrals

For GPs & referrers

Thank you for trusting us with your patients. Here is how to refer to IntegratePHYSIO, what we accept, and how we keep you informed.

IntegratePHYSIO — For GPs & referrers

Why refer to IntegratePHYSIO

  • Clinical leadership: led by Sam Bhojwani, Australia's first APA triple-titled physiotherapist, and Yamini Kapoor, APA Titled Continence & Pelvic Health Physiotherapist
  • Physiotherapy and exercise physiology under one roof, so your patient's care is coordinated from assessment through to long-term self-management
  • Same-day and after-hours appointments at Warner and Narangba, including private women's health suites
  • Structured programs including GLA:D®, the Better Back program, ACL rehabilitation and return to sport testing
  • Hydrotherapy, clinical Pilates and a fully equipped rehabilitation gym

Referrals we accept

  • Medicare GP chronic condition management plans: eligible patients can receive up to five Medicare-supported allied health services per calendar year (ten for Aboriginal and Torres Strait Islander patients). A referral letter is all we need; it does not need to name a specific provider. Referrals under existing GP Management Plans and Team Care Arrangements continue to be accepted while they remain valid.
  • DVA: for eligible Veteran Card holders, with a current DVA referral
  • WorkCover Queensland: work-related injuries, with an accepted claim number
  • CTP: motor vehicle accident injuries, with an approved claim
  • NDIS: we are a registered NDIS provider for physiotherapy and exercise physiology
  • Specialists and surgeons: pre- and post-operative rehabilitation, following your protocol
  • Private patients: no referral is needed, but we always welcome a letter with relevant history

How to refer

Send your referral securely via Medical Objects (search for IntegratePHYSIO), email it to the clinic your patient will attend, call us, or give the letter to your patient to bring to their first appointment. Helpful information includes:

  • Patient name, date of birth and contact details
  • Reason for referral and working diagnosis
  • Relevant medical history, medications and precautions
  • Imaging and test results
  • Surgical details and post-operative protocol, if applicable
  • Funding type (Medicare plan, DVA, WorkCover, CTP or NDIS) and claim numbers

Keeping you informed

We see ourselves as part of your patient's care team. After the initial assessment we send you a summary of our findings and treatment plan, with progress updates as required for the referral type, and a discharge summary at the end of care. If we identify anything that needs medical review, or your patient is not progressing as expected, we will contact you promptly.
